Step 1: Book Your Appointment
Active and Expired renewal appointments can be done virtually with Provider's/RMP's you have previously met in person
In person appointments are required for:
Patients seeking to obtain a New Card and have not had a Utah Medical Cannabis Card in the past
Patients renewing their active or expired cards who are transferring from another Provider/RMP or Company

Step 2: Fill Out and Submit Our Form, Sent via Email
The intake form will contain information we need to process your application. Please fill it out to the best of your knowledge, we are here to help if you need us.

Patients using a PTSD Diagnosis for their Qualifying Condition must submit documentation, within the intake form, from their Psychiatrist/Psychologist that is monitoring their PTSD

Step 3: Register with the Electronic Verification System (EVS)
visit evs.utah.gov to access the system
We will send instructions the day of your appointment to assist with this process. We also offer guidance for this step, if needed, during your appointment
For patients already registered with the Electronic Verification System, please login to your account and verify your status is "Awaiting Certification", we can assist with this step during your appointment as well
Step 4: Meet with One of Our RMP's
During your appointment, our provider will go through your intake form with you, answer any questions you have, and assist with certifying you for your Medical Cannabis Card
We will ensure that you have your Active Medical Cannabis Card and Dispensary Instructions before you leave us
